OneCare, a partnership led by regional physicians and Deaconess Health System, focuses on value-based care initiatives enhancing patient outcomes while managing healthcare costs. Through this collaboration, Fairfield Memorial Hospital will work alongside other providers to streamline care, improve access to essential services and strengthen community health efforts.

OneCare provides doctors and their patients with access to an extended care team of nurses and healthcare professionals who offer health guidance, encouragement and education between doctor appointments. Through an approach called Care Advising, OneCare ensures patients receive coordinated, high-quality care that is personalized, organized and focused on knowing them as people—not just patients.
Patients whose primary care doctors participate in OneCare may be contacted by a registered nurse Care Advisor to help manage their health more effectively. Care Advisors collaborate with physicians, case managers, pharmacists, dietitians and other healthcare professionals to provide resources and support for patients, especially those with chronic conditions or complex medical needs.
By working together to improve care coordination and reduce unnecessary medical services, OneCare participants help ensure patients receive the right care at the right time and at the right place while avoiding duplication of services and preventing medical errors.
