CARMI MANOR FINED $25,000 FOR MEDICATION FAILURES RESULTING IN PATIENT SEIZURES

Carmi Manor Rehabilitation and Nursing Center has been fined $25,000 by the Illinois Department of Public Health (IDPH) for serious failures related to patient medication management, according to the agency’s first quarter 2025 disciplinary report released this week.
The 74-bed facility, located at 615 Webb Street in Carmi, was found to have failed to obtain scheduled medications from the pharmacy and secure emergency medications, which led to a patient missing or abruptly stopping seizure medication. As a result, the patient suffered seizures, the report stated.
Carmi Manor has a history of violations and complaints. Since 2022, the facility has been fined a total of $227,464 related to 25 separate complaints, with four complaints filed so far in 2025 and 16 in 2024.
According to public health records, Carmi Manor is owned by five shareholders, with Baruch Sheps of Stern Consultants in New York holding a 40% majority stake. Other shareholders include Chaim Millman (25%), Eric Newhouse (15%), Bezalel Stern (15%), and Avraham Erblich (5%).
Stern Consultants also manages nursing homes in Robinson, Marshall, Ridgway, Macomb, Lacon, Henry, and Moravia, Illinois.
