MEDICARE RELEASES NEW RATINGS FOR AREA SKILLED CARE AND NURSING FACILITIES

Medicare has released its latest quality ratings for skilled care and nursing facilities, with Fairfield Memorial Hospital’s Skilled Care Unit earning a top score. The unit received a five-star rating—the highest possible—based on health inspections, staffing levels, and multiple quality-of-care measures.
Other facilities in the region did not score as highly.

Fairfield Senior Living and Rehabilitation LLC, located next to Fairfield Memorial Hospital, received a one-star rating, reflecting “much below average” performance in both staffing and health inspections.
Carmi Manor, located at 615 W. Webb near the former Carmi Township Hospital, also received a one-star rating. Medicare additionally flagged the facility for a recent citation involving abuse of a resident.
Cisne Rehabilitation received a three-star, or average, rating. Wabash Senior Living in Carmi likewise earned three stars, though it was marked “above average” in the quality category.

In Grayville, Haven on the River—formerly known as Meadowbrook—was issued a three-star rating. Oakview Nursing and Rehabilitation in Mount Carmel received a below-average two-star rating, while Silver Foxes Senior Living and Rehab in McLeansboro earned an average three-star ranking.

The Medicare star-rating system is updated regularly and is designed to help families compare nursing facilities based on federal inspection results, staffing data, and resident-care outcomes.
