NTSB RELEASES FINAL REPORT ON DEADLY TEUTOPOLIS, ILLINOIS CRASH INVOLVING ANHYDROUS AMMONIA

The National Transportation Safety Board (NTSB) has released its final report on the catastrophic highway crash that occurred near Teutopolis, Illinois, on September 29, 2023, resulting in five deaths and multiple injuries after a hazardous material spill.

According to the NTSB’s investigation, the incident took place around 8:41 p.m. on U.S. Highway 40 when a minivan, driven by a 17-year-old, attempted an unsafe passing maneuver in a no-passing zone. As the minivan tried to overtake a truck-tractor cargo tank combination vehicle carrying anhydrous ammonia, an oncoming vehicle approached. To avoid a head-on collision, the truck driver steered right, causing the vehicle to leave the roadway, enter a drainage channel, overturn, and slide into a parked utility trailer. The impact punctured the cargo tank, releasing anhydrous ammonia gas.
The toxic release led to five fatalities among local residents and vehicle occupants, nine serious injuries, and four minor injuries.

The NTSB determined that the probable cause of the crash was the teen driver’s unsafe passing maneuver, which forced the combination vehicle to take evasive action and lose control. The severity of the incident was compounded by the cargo tank’s collision with the utility trailer, which caused the hazardous material to be released.

The report highlights two primary lessons:
The crash underscores the dangers of inexperience among teen drivers, especially during complex or high-risk maneuvers like passing in poor visibility conditions. The NTSB urges parents, educators, and driving instructors to emphasize conservative driving decisions and awareness of one’s own limitations, even after formal driver education.

The incident also serves as a stark reminder of the dangers posed by anhydrous ammonia. Emergency responders are advised to use proper personal protective equipment (PPE) and decontamination procedures when dealing with such materials and to coordinate closely with hazardous material shippers for safe response and mitigation
