Horizon Healthcare in Fairfield has been awarded the Target: BP Silver+ Achievement Level by the American Heart Association (AHA) and the American Medical Association (AMA), recognizing the organization’s outstanding commitment to improving blood pressure control and promoting cardiovascular health. The Target: BP program is a national collaboration between the AHA and AMA designed to help healthcare providers improve blood pressure management and reduce the number of Americans who experience heart attacks and strokes.
