AREA NURSING HOMES FINED BY STATE FOLLOWING HEALTH VIOLATIONS

Two Southern Illinois nursing facilities have been fined a combined total of more than $50,000 following investigations by the Illinois Department of Public Health.

Haven on the River received fines totaling nearly $30,000 for multiple violations. The largest penalty, $25,000, stemmed from improper post-fall assessment and treatment of a female resident who suffered serious injuries.

According to investigation documents, the resident fell and sustained fractures to her hip and femur. Staff allegedly failed to properly assess the injuries, delaying necessary medical treatment. A family member later noticed the resident’s leg “didn’t look right,” prompting a hospital visit where imaging confirmed the fractures. The facility was also fined $2,200 after a resident fell from a lifting sling and struck her head, resulting in a laceration. In a separate incident, another $2,200 fine was issued after a resident did not receive prescribed pain medication, including a fentanyl patch, for an extended period described as “weeks.”

Meanwhile, Silver Foxes Senior Living and Rehabilitation was fined $25,000 for failing to provide adequate treatment to three residents suffering from urinary tract infections.

In one case, a resident developed sepsis and had to be airlifted to an area hospital. The individual was later treated and discharged to hospice care.
