A Grayville nursing home has been fined $50,000 for two separate healthcare violations, including one involving the death of a resident. According to the Illinois Department of Public Health’s second-quarter nursing home violation report, Haven on the River in Grayville received two fines of $25,000 each for failures involving resident care and monitoring.
In the first case, state inspectors determined the facility failed to follow physician’s orders and properly assess a resident’s declining medical condition. The resident, who had been admitted in December 2025 with heart disease, chronic obstructive pulmonary disease and diabetes, was later hospitalized with acute kidney injury and dangerously elevated potassium levels. The resident required emergency dialysis and subsequently died.
In the second case, IDPH determined the facility failed to properly assess and monitor another resident’s changing condition. That resident was hospitalized with sepsis resulting from a urinary tract infection, Influenza A and acute respiratory failure.
Haven on the River is operated by Extended Care Consultants, LLC, headed by Levi Israel of Chicago.
Meanwhile, Chestnut Corner, a shelter care facility in Louisville in Clay County, was fined $14,700 for numerous building maintenance violations. State inspectors documented water leaking through a basement ceiling, overflowing trash containers, bowing walls, missing dining room tiles and mold in shower areas.
IDPH reported that the maintenance deficiencies had the potential to affect all 73 residents at the facility.
Chestnut Corner is operated by the Zink family of Clay County.
The violations and fines were included in the state’s second-quarter inspection report.
